Nullius in verba .

For much of human history, life unfolded with a measure of continuity. An ancient farmer or a medieval craftsperson might have reached old age in a world that, in many of its everyday practices, remained recognisable from childhood. The knowledge handed down by previous generations often provided a useful guide to the lives of those who followed. Collective memory could help an individual find a place in the world.

Continuity, however, never meant immobility. Epidemics, wars, migrations and new ideas could transform entire communities within a single lifetime. The past was not a motionless landscape. Yet in many times and places, change in the ordinary patterns of life was slow enough for inherited experience to retain much of its value.

Our age presents a different challenge. We encounter not only events that alter the course of history, but also successive changes in the tools we use, the knowledge we rely on and the ways we relate to one another. Within a single lifetime, much that once seemed familiar may have to be learned anew.

This pace can leave us bewildered. The human mind has a remarkable capacity to adapt, but adaptation takes time, attention and experience. When new possibilities arrive before we have understood the consequences of earlier ones, the challenge is not merely to keep up. It is to preserve our ability to judge.

Here lies one of the tensions of our time. Information is more accessible than ever, yet that very abundance makes it harder to decide what deserves our attention. Scientific publications continue to multiply, and some technologies spread from country to country far more quickly than their predecessors. But more information does not necessarily bring deeper understanding. Nor does the rapid circulation of an idea tell us whether it is sound, useful or beneficial.

We must therefore distinguish information from knowledge, and knowledge from wisdom. Information can be gathered; turning it into knowledge requires evidence, context and careful interpretation. Wisdom asks a further question. What should we do with what we know? That question cannot always be answered at the speed with which new information reaches us.

The most thoughtful form of adaptation is not a frantic effort to embrace every innovation. Novelty and progress are not the same thing. A new tool deserves to be judged by what it makes possible, what it demands of us and what, perhaps, it displaces. Sometimes adapting well means changing our habits. At other times, it means recognising the value of something worth preserving.

This has consequences for what we pass on to future generations. Our ancestors did not possess a collection of immutable certainties, and neither do we. But when familiar answers may quickly become insufficient, it matters all the more that we pass on habits of mind. We can teach those who follow us to ask searching questions, weigh evidence and remain open to correction. Experience remains a form of wealth, even when it cannot supply a definitive answer to every new problem.

To live amid continuing change is demanding. It also offers a rare kind of wonder. We can see, within the span of our own lives, how human understanding is revised, extended and sometimes transformed entirely. Such a vantage point should make us wary of absolute certainty without leading us to abandon the search for truth. We may never stand outside the current and see the whole river at once. Still, we can learn to navigate it with curiosity, humility and care.

Nullius in verba

Nullius in verba

Glucose is essential for life, but when exposure remains excessive for too long, it can leave a molecular imprint on our tissues and blood vessels. Diabetes has a diagnostic threshold, but biology does not. Understanding this difference may help us protect our metabolic health long before a number becomes a diagnosis.

La entrada The Sweet Memory of Blood se publicó primero en El Rincón de Vag.

Let us think of the bloodstream not simply as a river of life, but also as an archive of our metabolic history. Within it, millions of red blood cells travel continuously through an intricate network of vessels, carrying out one of the body’s most essential tasks: delivering oxygen to living tissues.

These cells move through a medium containing glucose, one of the principal fuels used by our cells. But there is an important point to remember. The glucose circulating in our blood does not come only from sweet foods.

The carbohydrates in our diet — found in foods such as bread, rice, pasta, potatoes, legumes, cereals, and fruit — are broken down during digestion into molecules that can raise blood glucose, the amount of glucose circulating in the bloodstream.

There is another important source. Our liver can produce and release glucose when the body needs it. Even when we are not eating, this process helps maintain the blood glucose concentration required to supply tissues that depend particularly on glucose as an energy source.

Blood glucose is therefore the result of a delicate balance between what we absorb, what our body produces, and what our tissues use. As with so many biological processes, health depends largely on maintaining that balance.

The Chemistry of the Encounter: There is a common misconception that glucose begins attaching itself to our proteins only when metabolism becomes disturbed. In reality, glycation — the spontaneous attachment of glucose molecules to proteins and other molecules — is a continuous chemical process that occurs throughout the body.

Glucose can react with proteins, lipids, and other molecules without the involvement of an enzyme, meaning without the need for a specialized protein to catalyze the reaction.

What changes when glucose levels rise and remain elevated is the rate and extent of these reactions.

Glycation occurs in everyone, even under normal metabolic conditions. But there is a fundamental principle. The greater and more prolonged the exposure to glucose, the greater the extent of glycation.

Hemoglobin in red blood cells provides a particularly clear example. A small proportion of the glucose in the bloodstream binds spontaneously to hemoglobin, the protein that enables red blood cells to transport oxygen, forming glycated hemoglobin, or HbA1c.

Because red blood cells circulate for approximately 120 days, HbA1c provides a record of prolonged glucose exposure. It does not capture blood glucose at a particular moment. Instead, it reflects average glycemia over approximately the previous two to three months, with greater influence from more recent weeks.

In this way, HbA1c allows us to read part of our metabolic history through a small molecular modification of hemoglobin.

The Silent Erosion: If hemoglobin is a relatively short-lived record of our metabolic past, other proteins have a much longer memory.

Among them are collagen and elastin, structural proteins that provide strength and elasticity to many tissues. Some of these proteins are renewed slowly and can remain in the body for years.

When glycation is excessive and prolonged, proteins undergo progressive chemical modifications and may accumulate advanced glycation end-products, commonly known as AGEs. These compounds are formed through a series of reactions associated with glycation and can alter the structure and function of tissues.

Some AGEs can form cross-links between proteins, changing their mechanical properties. Others can interact with cellular receptors and contribute to oxidative stress, an imbalance involving reactive molecules that can damage cellular components, and to chronic inflammation, a persistent inflammatory response that may contribute to tissue injury.

The image of tissues being “caramelized” may be evocative, but the biological reality is much more complex. Our tissues do not literally become caramelized. Rather, molecular modifications accumulate progressively and may alter the structure and function of proteins over time.

The consequences can affect different parts of the body.

The retina, the light-sensitive layer at the back of the eye, where damage to small blood vessels can contribute to diabetic retinopathy and impair vision.

The kidneys, whose microscopic filtering units, called glomeruli, can undergo structural and functional changes that contribute to diabetic kidney disease.

The peripheral nervous system, consisting of the nerves that connect the brain and spinal cord with the rest of the body. Metabolic and vascular disturbances can contribute to neuropathy, meaning nerve damage that may cause pain, tingling, or loss of sensation.

The heart and blood vessels, where chronic hyperglycemia — persistently elevated blood glucose — contributes to cardiovascular risk in combination with other metabolic disturbances such as insulin resistance, high blood pressure, abnormal blood lipids, and excess visceral fat.

The important point is that vascular damage in type 2 diabetes is multifactorial. Persistent hyperglycemia and the molecular changes associated with it are part of the picture, but they interact with several other metabolic and vascular factors.

The Road Ahead: Science offers us a sensible compass. Health does not require us to eliminate glycation, which is a natural chemical process that cannot be completely avoided. What matters is limiting excessive and prolonged exposure to glucose while preserving healthy metabolic function.

We can influence our metabolic health through everyday choices.

1. The Order of the Meal

Beginning a meal with vegetables and other fiber-rich foods can help moderate the rise in blood glucose after eating, particularly when carbohydrate-rich foods are consumed afterwards.

The overall composition of the meal, the amount and quality of carbohydrates, and the degree of food processing remain much more important than the order of individual foods. This is therefore not a formula for compensating for an otherwise unbalanced diet.

2. Movement After Eating

A walk or light physical activity after a meal encourages muscles to take up and use glucose and can reduce the postprandial glycemic response, meaning the rise in blood glucose that occurs after eating.

It is a simple intervention whose real value lies in making it part of a regular routine.

3. The Small Alchemy of Vinegar

Acetic acid, one of the principal components of vinegar, can modestly reduce the rise in blood glucose after a meal when vinegar is consumed with food.

Its effect, however, is limited and should never be regarded as a means of compensating for an unhealthy diet. The overall quality of what we eat remains vastly more important.

4. Restorative Sleep

Adequate sleep is an essential component of metabolic health. Sleep deprivation and disruption of our daily biological rhythms can impair insulin sensitivity, meaning the ability of cells to respond appropriately to insulin and take up glucose.

Good sleep helps the body regulate energy metabolism more effectively.

Diabetes Has a Threshold. Biology Does Not: Here we encounter a particularly important distinction.

Medicine needs objective criteria and defined thresholds to diagnose diabetes. Under current criteria, diabetes in nonpregnant adults can be diagnosed with an HbA1c of 6.5% or higher, a fasting plasma glucose of 126 mg/dL or higher, a two-hour plasma glucose of 200 mg/dL or higher during a 75-g oral glucose tolerance test, or a random plasma glucose of 200 mg/dL or higher in a person with classic symptoms of hyperglycemia or a hyperglycemic crisis. In the absence of unequivocal hyperglycemia, the abnormal result must be confirmed.

These thresholds are indispensable. They provide medicine with consistent criteria for diagnosis, guide clinical decisions, and allow meaningful comparisons among patients and studies.

But biology does not recognize diagnostic boundaries.

An HbA1c of 6.4% does not mean that the blood vessels are completely protected, while an HbA1c of 6.5% does not mean that vascular risk suddenly changes the following day. Metabolism does not operate like a switch that changes from “normal” to “diabetic” when a particular number is reached.

Risk follows a continuum. An HbA1c of 5.8% does not meet the diagnostic criteria for diabetes, but it does not represent exactly the same metabolic risk profile as an HbA1c of 5.0%. The ADA recognizes that the relationship between HbA1c and the risk of developing diabetes is continuous and curvilinear, extending below the conventional prediabetes range.

This distinction is fundamental to understanding the disease.

We can speak of diabetes as defined by diagnostic criteria and, separately, of metabolic deterioration that may precede the diagnosis by years.

These are two ways of looking at the same biological continuum. One belongs to the necessary language of medical diagnosis. The other reflects the gradual nature of biology.

And this has an important consequence. The body does not wait for HbA1c to reach 6.5% before responding to an unfavorable metabolic environment.

Hyperglycemia is one of the factors involved, but it does not act in isolation. Insulin resistance, high blood pressure, abnormal blood lipids, visceral fat accumulation, chronic low-grade inflammation, and other metabolic factors can interact well before a diagnosis of type 2 diabetes is established.

That is why waiting for a diagnosis before taking care of metabolic health may mean waiting until a biological process that has been developing for some time has crossed a clinical threshold.

A Reflection on the Future: Monitoring HbA1c can be an exercise in metabolic self-awareness. An HbA1c below 5.7% is within the range considered normal under current diagnostic criteria. Between 5.7% and 6.4% lies the range classified as prediabetes, indicating an increased risk of developing diabetes. An HbA1c of 6.5% or higher falls within the diagnostic range for diabetes.

But a single number does not define a person’s entire metabolic health.

HbA1c is a molecular footprint of prolonged exposure to glucose. That exposure depends on a system far more complex than the amount of sugar we eat.

Our structural proteins can have a much longer memory than our red blood cells. Some are renewed slowly and may accumulate molecular modifications over time. That is why taking care of our metabolism today also has consequences that reach into the future.

The blood, by contrast, is constantly renewed. Red blood cells that leave the circulation are replaced by new ones, and each generation carries its own metabolic history.

Reducing sustained hyperglycemia decreases excessive glucose exposure and therefore reduces one of the conditions that promote the formation and accumulation of glycated proteins. We cannot immediately erase the molecular marks of the past, but we can avoid adding to them.

Ultimately, caring for our metabolic health means preserving a delicate balance between flexibility and rigidity, renewal and wear. It means protecting our blood vessels, tissues, and organs from a metabolic environment that, when unfavorable for long enough, can leave an increasingly deep molecular imprint.

This is not about giving up the sweetness of life. It is about understanding that glucose is part of a complex energy economy that the body regulates continuously.

Because true metabolic harmony does not mean eliminating glucose, an essential fuel for life.

It means preventing excessive and sustained exposure from becoming a burden on our biology and mortgaging our future.

In the field of health, the relationship between body weight and cancer has been the subject of extensive scientific research. Excess body weight is associated with an increased risk of several chronic diseases and with a higher risk of developing certain types of cancer. But what happens when a person begins to lose weight without having deliberately changed their diet or level of physical activity? This question lies at the heart of today’s reflection.

Unintentional weight loss, particularly when it is persistent or substantial and cannot be readily explained, deserves medical attention. It is a recognized symptom of a wide range of underlying conditions, including some forms of cancer. It is important to emphasize, however, that weight loss is not in itself an indication of cancer. Rather, it is a nonspecific clinical sign that may indicate that something is wrong and warrants appropriate medical evaluation.

Several types of cancer are frequently associated with unexplained weight loss. Pancreatic, gastric, esophageal, and lung cancers, among others, are relatively often associated with this symptom. Weight loss may occur at different stages of the disease and can result from several interacting mechanisms, including reduced appetite, early satiety, changes in taste or smell, difficulty eating or swallowing, and the metabolic effects of the disease itself.

But how exactly can cancer contribute to weight loss in the absence of deliberate changes in lifestyle? The answer lies, at least in part, in the complex ways in which cancer can alter the body’s metabolism. Tumors and the body’s response to them can trigger inflammatory and hormonal changes that affect appetite, energy expenditure, and the metabolism of proteins, fats, and carbohydrates. In some patients, these processes contribute to a condition known as cancer cachexia, characterized by progressive loss of skeletal muscle and, often, body fat. Importantly, this process cannot be explained simply by eating less, because profound metabolic alterations may occur even when food intake is maintained.

It is therefore important not to ignore unexplained weight loss or automatically attribute it to external factors such as stress or anxiety, although these may indeed contribute. Persistent or significant unintentional weight loss should be discussed with a physician, particularly when it is accompanied by other symptoms such as persistent fatigue, changes in bowel or bladder habits, difficulty swallowing, persistent pain, fever, night sweats, jaundice, or a sustained loss of appetite.

That said, it is equally important to recognize that unintentional weight loss is far from synonymous with cancer. Many other conditions can cause a decrease in body weight without any deliberate change in lifestyle. These include gastrointestinal disorders, endocrine diseases such as hyperthyroidism, diabetes, chronic infections, medication-related effects, and psychological conditions such as depression. Sometimes, the cause is much less serious. Nevertheless, unexplained weight loss deserves attention precisely because its causes are so varied.

For long-term health, maintaining a healthy body weight and reducing the risk of chronic disease, including several forms of cancer, are supported by a balanced lifestyle that combines a nutritious diet, regular physical activity, adequate sleep, and avoidance of tobacco and excessive alcohol consumption. These measures contribute to overall health and help reduce the risk of many preventable diseases.

Unexplained weight loss can be a troubling symptom, but it is not a diagnosis. In some cases, it may be associated with cancer; in many others, it has an entirely different cause. What matters is not to ignore an unexpected and persistent change in body weight, but to understand why it is occurring.

Do not succumb to the temptation of thinking, “It’s probably nothing.” Instead, let your physician determine whether there is anything that requires attention. Sometimes, the reassurance that follows an appropriate medical evaluation is itself a form of good health care.

Your health is among the most valuable things you possess, and paying attention to the signals your body sends you is an essential part of caring for it.

Nullius in verba

Quizá nuestra sociedad necesite recuperar precisamente esa flexibilidad.

“Difficult things are achieved through effort; impossible things through intelligence.”

The saying is often attributed to Thales of Miletus, the sixth-century BCE Ionian thinker whom many regard as the first philosopher of the Western tradition. The attribution, however, deserves caution. Many sayings from the ancient world have reached us without a direct source that would allow us to establish with certainty who first uttered them.

Yet perhaps the identity of its author matters less than the fact that the saying has endured for twenty-five centuries without losing its power to speak to us. Perhaps this is because it expresses a simple truth that we continue to recognise in our own lives.

To understand it, imagine water meeting stone.

Water can wear away rock without extraordinary force. Its strength lies in persistence. Drop by drop, year after year, century after century, it transforms what once seemed immovable. It does not overcome the stone with a single blow. It changes it because it never ceases to act upon it.

That is effort.

But water has another strategy. When it finds a crack, however narrow, it does not need to keep striking the surface. It enters quietly, following a path where, only moments before, none seemed to exist.

That is intelligence.

To persevere until an obstacle gives way, or to understand it well enough to find another route through it. These are two distinct ways of confronting difficulty, and our society would do well to recover a wiser balance between them.

The science of learning helps us understand the first.

Repeated practice, when undertaken with attention and supported by appropriate feedback, gradually alters the neural circuits involved. Synaptic plasticity, the capacity of connections between neurons to change with experience, allows certain pathways to strengthen. This contributes to greater efficiency in performing learned skills. Sustained practice may also be associated with changes in the myelination of specific neural pathways, helping signals travel faster and with greater coordination.

This is how much of what we call experience is formed.

Someone who masters a musical instrument, a language, a scientific discipline, or a craft does not do so through innate talent alone. Competence is built through thousands of apparently insignificant acts which, accumulated over time, reshape the way the brain processes what it has learned.

Water wears away stone.

But there is another way forward.

In problem-solving, there are moments when further effort brings us no closer to a solution. Then, suddenly, the way we see the problem changes. A relationship that had remained hidden becomes visible. What seemed to be a dead end reveals an unexpected opening.

Cognitive psychology refers to this form of sudden understanding as insight.

It does not necessarily mean thinking more. It means thinking differently. Changing the frame. Questioning an assumption. Looking from another angle. Sometimes an obstacle does not disappear because we have become stronger, but because we realise that we have been trying to overcome it from the wrong direction.

The history of thought had already recognised this distinction.

Aristotle distinguished between technē, practical knowledge acquired through practice, and phronesis, a form of practical wisdom concerned with deliberation and sound judgement about what ought to be done in a particular situation.

One enables us to perfect a skill.

The other helps us decide which skill is worth using.

Our age, however, seems to have elevated effort, understood as sheer quantity, almost to the status of a virtue.

The so-called hustle culture, visible in much contemporary discourse on professional success, has turned constant activity into a badge of worth. Work more. Produce more. Reply faster. Sleep less. Exhaustion can become a sign of commitment, while the number of hours invested is treated as an almost automatic measure of merit.

But being busy does not necessarily mean moving forward.

Research on burnout has documented the consequences of prolonged occupational stress without adequate recovery. The problem is not effort itself. Effort is indispensable. The problem begins when we stop asking whether we are directing it where it can do the most good.

We can spend years striking the same stone.

And there may be a crack only a few inches away.

At the same time, we are living through a technological revolution that is changing the relative value of effort and knowledge.

Artificial intelligence can process, in seconds, quantities of information that would overwhelm any individual mind. Scientific networks allow thousands of researchers to share data, question findings, and build knowledge together. Problems that once demanded vast amounts of accumulated time can now be approached with tools that dramatically extend our cognitive capacities.

This is changing our understanding of intelligence itself.

For centuries, we admired the individual who knew more than anyone else. Today, it is increasingly clear that an essential part of intelligence lies in connecting knowledge, asking meaningful questions, detecting errors, and thinking with others.

Collective intelligence does not diminish individual intelligence.

It amplifies it.

Yet this creates a new responsibility. The more powerful our tools become, the more important it is to know what to ask, how to interpret an answer, and when to question it. Computational speed is not the same as depth of judgement. And having an answer does not mean that we understand the problem.

Perhaps the greatest educational challenge of our time, then, is not to teach children to accumulate ever more information.

It is to teach them how to think when information is increasingly available at their fingertips.

An education based exclusively on repetition can produce disciplined people who are capable of solving familiar problems. Yet the world changes precisely when someone dares to ask a question for which there is still no answer.

We need perseverance, but we also need curiosity.

We need memory, but we also need imagination.

We need to learn how to solve problems, but also how to ask whether we are trying to solve the right problem.

There is, however, a more uncomfortable question that belongs in this reflection.

Not everyone faces the same stone.

Nor does everyone have the same opportunity to discover its cracks.

Where we are born, the quality of the education we receive, the people we encounter, an unexpected conversation, a teacher who awakens a dormant curiosity, or even a seemingly insignificant circumstance can profoundly alter the course of a life.

Chance, too, plays a part in our story.

Recognising this does not diminish the value of effort or intelligence. It simply makes us more cautious when we speak of merit. Not everyone is given the same stone, the same tools with which to work on it, or the same opportunity to notice where its cracks lie.

A truly advanced society should aspire to more than creating opportunities for people to work hard. It should strive to give everyone a genuine opportunity to learn how to look.

Perhaps three lessons follow.

The first is simple. We should not confuse effort with effectiveness. There are times when perseverance is the only answer. There are others when continuing to do the same thing means nothing more than persevering in the wrong direction.

The second concerns artificial intelligence. It does not free us from the need to think. It raises the level of thinking required of us. When a machine can provide an answer in seconds, human value shifts towards the question, the judgement, and the ability to distinguish what is useful from what is merely plausible.

The third may be the most important. Merit needs opportunity in order to reveal itself. A just society cannot guarantee that everyone will reach the same destination. It can, however, strive to ensure that no one is denied the tools needed to discover how far they might go.

Perhaps the saying attributed to Thales contains a deeper lesson than first appears.

It is not a matter of choosing between effort and intelligence.

It is a matter of knowing when each is needed.

Effort teaches us the nature of the stone. Intelligence helps us realise that perhaps we do not need to break through it at all.

And, paradoxically, the two depend upon one another. Someone who has never persevered is unlikely to know an obstacle well enough to understand it. But someone who knows only how to persevere may eventually mistake resistance for progress.

Water has no single strategy.

It strikes when striking is necessary. It seeps through when it finds a crack. It flows around an obstacle when the terrain permits. And when it finds a channel, it follows it.

Perhaps our society needs precisely that kind of flexibility.

To persevere when perseverance is worthwhile.

To stop when pushing harder no longer makes sense.

To change our perspective when the path closes.

And to learn to look at the stone before striking it again.

Because true intelligence does not always lie in finding the strength to overcome an obstacle, but in knowing when to stop fighting it and look for another way.

Fecal odor is rarely considered an appealing subject, yet it offers a revealing window into the complexity of digestion and the activity of the gut microbiota. Often dismissed as an unpleasant curiosity or reduced to a source of humor, it is, in fact, the result of a dynamic interaction between diet, intestinal transit, microbial metabolism, and host physiology.

The characteristic odor of feces arises largely in the colon, where a dense and metabolically diverse community of microorganisms transforms dietary components that have escaped digestion and absorption in the small intestine. This activity generates a broad range of metabolites and volatile compounds. Among the most relevant are volatile sulfur compounds, indoles such as indole and skatole, phenols, ammonia, biogenic amines, and other products of microbial metabolism.

These compounds do not contribute equally to odor. Some are detectable by the human nose at extremely low concentrations and may therefore have a disproportionately strong effect on the final smell. Volatile sulfur compounds, for example, often have particularly low odor thresholds. Indole and skatole contribute to the characteristic fecal note, while ammonia and certain amines may add pungent or unpleasant nuances.

Intestinal fermentation also produces gases such as carbon dioxide and hydrogen. In some people, methane is generated as well, mainly through the activity of methanogenic archaea rather than bacteria. These gases are important in relation to bloating, abdominal distension, and flatulence, but they are not usually the principal determinants of fecal odor because carbon dioxide, hydrogen, and methane are odorless.

Diet and microbial metabolism: Diet is one of the main influences on fecal odor because it determines which substrates reach the colon and become available to intestinal microorganisms. Foods rich in sulfur-containing compounds, including eggs, certain meats, and cruciferous vegetables such as broccoli, cabbage, and cauliflower, can alter the production of sulfur metabolites during microbial fermentation.

Protein intake may also influence the production of odor-active compounds. When dietary proteins and amino acids reach the colon in greater quantities, microbial metabolism can produce ammonia, amines, phenols, indoles, and sulfur-containing metabolites. By contrast, fermentable carbohydrates tend to promote saccharolytic fermentation, meaning the microbial metabolism of carbohydrates. This process is often associated with the production of short-chain fatty acids, including acetate, propionate, and butyrate.

The relationship is not as simple as saying that fruit and vegetables always produce milder-smelling stools, or that protein invariably causes a stronger odor. The final result depends on the overall dietary pattern, the amount and type of fiber consumed, the balance between carbohydrate and protein reaching the colon, intestinal transit time, and the metabolic characteristics of each person’s microbiota.

For this reason, a temporary change in fecal odor after a particular meal, a period of higher protein intake, an increase in cruciferous vegetables, or a sudden change in fiber intake is usually unsurprising. It may simply reflect a short-term shift in the substrates available to intestinal microorganisms.

When odor deserves attention: Fecal odor alone is not a diagnostic test. It is neither specific enough nor reliable enough to identify a particular digestive disorder in isolation. Nevertheless, a sudden and persistent change in odor can be worth noticing, especially when it occurs together with other symptoms.

Medical assessment is appropriate when a persistent change in stool odor is accompanied by diarrhea that does not resolve, significant or persistent constipation, abdominal pain or marked bloating, blood in the stool or black, tar-like stools, fever, unintentional weight loss, oily or greasy stools, unusually pale or floating stools, or a marked and sustained change in stool frequency or consistency.

These features can occur in a range of conditions, including gastrointestinal infections, malabsorption, pancreatic insufficiency, inflammatory bowel disease, celiac disease, and other digestive disorders. However, none can be diagnosed from stool odor alone. Odor must always be interpreted alongside symptoms, medical history, diet, medication use, and, when necessary, clinical or laboratory evaluation.

A useful practical principle is simple. A brief change after dietary variation is commonly benign. A change that persists despite a stable diet deserves closer attention, particularly when it appears together with other digestive or systemic symptoms.

Aging, transit, and the microbiota: Aging adds another layer of complexity. The gut microbiota changes throughout life, but those changes are shaped by much more than chronological age. Diet, physical activity, medication, oral health, immune function, chronic disease, living environment, bowel habits, and hospitalization can all influence microbial composition and metabolic activity.

In older adults, studies often report changes in the abundance of bacterial groups associated with the fermentation of dietary fiber and the production of short-chain fatty acids. These changes are not universal, however, and should not be interpreted as an inevitable decline in intestinal health. Healthy aging can be accompanied by the preservation of a resilient, diverse, and metabolically active microbial ecosystem.

It would therefore be inaccurate to suggest that older people have a single or predictable pattern of fecal odor. Age may influence the intestinal environment, but it does not determine the smell of stool in any simple or universal way. The same is true of constipation and intestinal transit.

Constipation becomes more common with age for many reasons, including lower physical activity, inadequate fluid intake, medications, dietary changes, pelvic-floor dysfunction, and certain chronic diseases. When intestinal transit is prolonged, food residues remain in the colon for longer, and the water content of stool often changes. These factors can alter microbial metabolism and the concentration of odor-active compounds.

Yet slower transit does not automatically mean stronger-smelling stools. The outcome depends on the available substrates, the degree of water reabsorption, the composition of the microbiota, and the specific metabolites produced. The relationship between transit time and fecal odor is therefore real but complex.

Fiber, lactulose, and lactitol: Dietary fiber plays an important role because it provides substrates for intestinal microorganisms. Different fibers have different physical structures and are fermented to different degrees. Some reach the colon largely intact, where they can be metabolized by microorganisms and contribute to the production of short-chain fatty acids.

These short-chain fatty acids are not merely fermentation by-products. They contribute to the colonic environment, influence intestinal pH, and can affect microbial activity. Butyrate is particularly important because it serves as a major energy source for colonocytes, the cells lining the colon. It also contributes to the maintenance of the intestinal epithelial barrier and participates in local immune and inflammatory regulation.

The production of butyrate varies substantially between individuals. It depends on the type of fermentable substrate consumed, the rate of intestinal transit, the amount of substrate reaching the colon, and the presence of microorganisms capable of producing butyrate or supporting its production through metabolic interactions with other bacteria.

This is why dietary fiber should be regarded as a fundamental component of a healthy approach to colonic fermentation. A varied intake of fiber-rich foods, increased gradually and according to individual tolerance, generally provides a more appropriate long-term strategy than attempting to manipulate the microbiota through isolated products.

Lactulose and lactitol are different from ordinary dietary fiber, although both reach the colon and can be fermented by intestinal microorganisms. They are poorly absorbed carbohydrates with osmotic laxative effects. By drawing water into the intestinal lumen, they soften stools and facilitate bowel movements, particularly in people with constipation.

They may also modify microbial activity. Lactulose, in particular, has been associated in some studies with increases in bifidobacteria and changes in microbial fermentation products. Lactitol may have related effects, but the two compounds should not be considered interchangeable. Their impact depends on dose, baseline bowel habits, diet, medication use, and the individual composition of the microbiota.

A small but meaningful clue: Fecal odor reflects, imperfectly but sometimes instructively, the interaction between what we eat, what reaches the colon, how our gut microbiota transforms it, and how our digestive system functions.

This is why we should pay attention not merely to the smell of our feces, but to changes in that smell over time. A sudden alteration does not necessarily mean that something is wrong. Sometimes it is simply the chemical consequence of a particular meal or a temporary change in our diet.

The more interesting situation arises when the change persists despite an apparently stable diet. Has bowel frequency changed? Is the stool now looser, harder, greasier, paler, or more difficult to pass? Has bloating, abdominal discomfort, fever, or weight loss appeared? These observations should not be considered separately from the change in odor. Taken together, changes in fecal odor, stool characteristics, bowel habits, and associated symptoms can provide a much more informative picture of what may be happening in the digestive system than any one of these signs considered in isolation.

In this sense, a change in odor may be the signal that prompts us to look more closely. It may lead us to ask what has changed in our diet, whether our bowel habits are different, and whether other digestive symptoms have appeared. Sometimes the explanation is simply a particular meal or a temporary dietary change. At other times, when several changes persist together, they may indicate that something has altered in the intestinal environment and deserves further attention.

We can also ask a simpler question. What has changed? Have we increased our protein intake? Have we introduced a new food or supplement? Have we altered the amount or type of fiber in our diet? Is the change transient, or does it persist?

These observations encourage us to understand the intestine as a dynamic ecosystem in which food, microbiota, and host physiology are constantly interacting. Providing the microbiota with appropriate fermentable substrates through an adequate intake of dietary fiber and, when medically appropriate, non-digestible carbohydrates such as lactulose or lactitol can influence microbial metabolism and the production of short-chain fatty acids.

The smell of feces, therefore, is not a diagnostic test and should never be interpreted in isolation. It is one of several subtle clues that can invite us to pay attention to the dialogue between our diet and our intestinal microbiota. Sometimes the message is simply that we have eaten something different. At other times, particularly when a change persists, it may prompt us to consider whether something has changed in our intestinal environment and in the way our microbiota processes the food we provide.

What may seem like an insignificant and embarrassing biological detail is, in fact, a small expression of the biochemical activity taking place in the colon every day. Fecal odor is not a diagnosis, but a change in its familiar pattern can be a subtle clue that invites us to pay attention. Sometimes it simply reflects what we have eaten. At other times, particularly when the change persists, it may encourage us to look more carefully at our digestive health.

Learning to notice these changes is, in its own small way, learning to listen to the intestine. Diet, intestinal transit, microbial metabolism, and digestive health are part of the same intricate biological conversation.

The human body is a master of concealment. For years, sometimes decades, it can sustain processes that gradually erode its resilience without issuing a single cry for help. We wake each morning with a sense of well-being that persuades us that the absence of pain is evidence of health. It is not. Health is not a static condition but a dynamic equilibrium, and disease is rarely a sudden event. More often, it is the culmination of biological changes that have been unfolding long before symptoms bring them to our attention.

A Storm Forming in a Clear Sky: One of the most common misconceptions is that a disease begins on the day a physician gives it a name. In reality, many diseases develop silently for years before they cross the threshold at which they become clinically recognizable.

Consider Alzheimer’s disease. The accumulation of amyloid-β plaques in the brain, a protein that clumps together and interferes with normal neuronal function, can begin many years, often one to two decades, before the first noticeable cognitive symptoms. During that long preclinical period, pathological changes may be advancing while everyday life appears entirely normal. The storm is already forming, even though the horizon still looks serene.

Type 2 diabetes follows a similar pattern. Long before blood glucose reaches the diagnostic threshold, the body may have endured years of metabolic stress. Tissues become progressively less responsive to insulin, while the pancreas increases insulin secretion to compensate and keep glucose levels within the normal range. By the time a blood test raises the alarm, the underlying metabolic disturbance may have been developing for years.

The heart, too, can remain remarkably silent. Autopsy studies conducted across several decades and populations, among them a well-known series performed on young soldiers who died in combat, revealed that many already had evidence of coronary atherosclerosis despite being young and apparently healthy. This finding, confirmed since in civilian populations of different countries, showed that the disease had begun long before symptoms could announce its presence. Decades may pass between the earliest biological changes and the first clinical warning.

It is a language we have often failed to learn to interpret.

The Trap of Red Lines: Here lies one of medicine’s great tensions, our dependence on diagnostic labels. The thresholds used to distinguish health from disease are indispensable for clinical practice, yet many of them represent points along a biological continuum rather than abrupt boundaries in nature.

Take blood glucose, as an example that illustrates the point well. A fasting glucose level of 99 mg/dL falls within the normal range, whereas 100 mg/dL meets the laboratory criterion for impaired fasting glucose, commonly classified as prediabetes. Does anything dramatic change in our cells because of that single milligram? Of course not. What changes immediately is the category, not the underlying biology.

This threshold effect can create a false sense of reassurance. A value just below a diagnostic cut-off may be interpreted as a green light, even when the broader pattern of metabolic health deserves attention.

Biology does not work in categories as neatly as medicine sometimes needs it to.

Listening to the Whisper: The good news is that many of these processes leave measurable traces, and modern medicine is increasingly capable of detecting them. The earlier a clinically meaningful change is recognized, the greater the opportunity to modify the factors that are driving it. Blood pressure, lipid profiles, glucose regulation, inflammatory markers and, in appropriate contexts, heart-rate variability, a measure of the tiny fluctuations between consecutive heartbeats that reflects how well the nervous system regulates the heart, can provide valuable information about physiological function.

They do not predict the future with certainty. But taken together, interpreted in context and followed over time, they can reveal patterns that a single measurement may conceal. They give us an opportunity to adjust our course before a biological disturbance becomes an established disease.

This opportunity, however, carries its own price. Listening too closely to the body’s whisper can also generate noise. Detecting a subtle change years before any disease might develop does not always translate into a clear course of action, and it can produce anxiety, unnecessary testing or treatments whose benefit is not yet established. Alzheimer’s disease is a case in point, since we are now able to detect amyloid accumulation long before any cognitive symptom appears, yet medicine still does not have a reliable way to prevent the disease in a person identified this way. Early detection is valuable, as a rule, but it is not automatically synonymous with benefit. The whisper deserves attention, not alarm, and knowing the difference is itself a form of medical wisdom.

The Twin Living in the Data: Imagine projecting your biology into a digital mirror, an evolving computational representation built from longitudinal health data such as laboratory measurements, physiological signals, medical history and, where scientifically validated and appropriately available, genomic or microbiome information. A model that does not feel, but can reveal patterns. This is the promise of a digital twin in medicine.

The concept originated in engineering, where digital representations of physical systems are used to monitor performance, anticipate failures and test scenarios. It is now being explored in medicine, where the ambition is to move from isolated measurements towards a more dynamic representation of an individual’s health.

A medical digital twin could integrate repeated observations and identify subtle changes that might appear insignificant when viewed separately but become meaningful when considered together. Its greatest potential lies not in replacing the physician or predicting an individual’s destiny, but in helping clinicians understand trajectories, personalize interventions and explore possible outcomes before decisions are made.

The idea of a virtual double that absorbs information in the digital realm to help protect its physical counterpart resonates with a deeply human desire, to foresee danger before it becomes suffering.

Yet this technology also casts shadows that deserve our attention. Who owns our most intimate biological data? Who controls how it is used? How do we prevent increasingly sophisticated medicine from becoming accessible only to those who can afford it? These questions must be addressed as carefully as the technology itself is developed.

Caring for the body before a diagnosis appears is not anxiety. It is a more mature way of inhabiting our own lives, provided that this vigilance is guided by evidence and proportion rather than by fear.

The body’s silence is not always a sign of peace. Sometimes it simply means that we have not yet learned to listen.

Its whispers may be subtle, but learning to hear them, and knowing when to act on them, can give us something extraordinarily valuable, time.
