Durante décadas, la farmacología ha actuado principalmente aguas abajo, tratando de reducir la presión cuando el cauce ya discurría con fuerza. Las nuevas moléculas proponen una estrategia diferente, </span></span><strong><span style="font-family: Garamond, serif;"><span style="font-size: large;">remontar el río hasta aproximarse a algunas de sus fuentes biológicas para interrumpir la señal antes de que se propague</span></span></strong><span style="font-family: Garamond, serif;"><span style="font-size: large;">.</span></span></p>
<p><span style="font-family: Garamond, serif;"><span style="font-size: large;">La magnitud del problema impresiona. </span></span><strong><span style="font-family: Garamond, serif;"><span style="font-size: large;">En 2024, unos 1.400 millones de adultos de entre 30 y 79 años vivían con hipertensión en todo el mundo</span></span></strong><span style="font-family: Garamond, serif;"><span style="font-size: large;">, según la Organización Mundial de la Salud. En este territorio de dificultad terapéutica se concentran algunas de las innovaciones farmacológicas más interesantes de los últimos años.</span></span></p>
<p><span style="font-family: Garamond, serif;"><span style="font-size: large;">La primera ruta se dirige contra la </span></span><strong><span style="font-family: Garamond, serif;"><span style="font-size: large;">aldosterona</span></span></strong><span style="font-family: Garamond, serif;"><span style="font-size: large;">, hormona que favorece la retención renal de sodio y agua y contribuye a elevar la presión arterial. Una proporción relevante de los pacientes con hipertensión resistente presenta una producción de aldosterona inapropiadamente elevada. Los </span></span><strong><span style="font-family: Garamond, serif;"><span style="font-size: large;">inhibidores de la aldosterona sintasa</span></span></strong><span style="font-family: Garamond, serif;"><span style="font-size: large;"> actúan directamente sobre la enzima responsable de su síntesis, en lugar de bloquear su receptor. La FDA lo aprobó el 15 de mayo de 2026 para el tratamiento de la hipertensión en combinación con otros antihipertensivos. <p><span style="font-family: Garamond, serif;"><span style="font-size: large;">La posibilidad de espaciar las administraciones tiene una relevancia clínica evidente. </span></span><strong><span style="font-family: Garamond, serif;"><span style="font-size: large;">La adherencia constituye uno de los grandes desafíos del tratamiento antihipertensivo</span></span></strong><span style="font-family: Garamond, serif;"><span style="font-size: large;">, especialmente cuando exige una medicación diaria durante décadas. Sustituir cientos de tomas anuales por dos administraciones programadas podría facilitar el cumplimiento terapéutico, aunque su verdadero impacto sobre la adherencia y, sobre todo, sobre los acontecimientos cardiovasculares todavía debe demostrarse.</span></span></p>
<p><span style="font-family: Garamond, serif;"><span style="font-size: large;">Completan este nuevo mapa </span></span><strong><span style="font-family: Garamond, serif;"><span style="font-size: large;">el aprocitentán</span></span></strong><span style="font-family: Garamond, serif;"><span style="font-size: large;">, antagonista de los receptores de endotelina aprobado en Estados Unidos en 2024 para pacientes cuya hipertensión no está adecuadamente controlada con otros tratamientos, y moléculas en investigación como </span></span><strong><span style="font-family: Garamond, serif;"><span style="font-size: large;">la ocedurenona</span></span></strong><span style="font-family: Garamond, serif;"><span style="font-size: large;">, un antagonista no esteroideo del receptor mineralocorticoide que ha mostrado reducciones de la presión arterial en pacientes con enfermedad renal crónica e hipertensión no controlada o resistente.</span></span></p>
<p><span style="font-family: Garamond, serif;"><span style="font-size: large;">Pero conviene mantener una perspectiva esencial. </span></span><strong><span style="font-family: Garamond, serif;"><span style="font-size: large;">La innovación farmacológica no desplaza los fundamentos del control de la presión arterial</span></span></strong><span style="font-family: Garamond, serif;"><span style="font-size: large;">, entre ellos una alimentación saludable, la reducción del consumo de sodio, la actividad física, el mantenimiento de un peso adecuado y la moderación del consumo de alcohol. <p><span style="font-family: Garamond, serif;"><span style="font-size: large;">Cada reducción de </span></span><strong><span style="font-family: Garamond, serif;"><span style="font-size: large;">5 mmHg en la presión arterial sistólica disminuye aproximadamente un 10 % el riesgo de acontecimientos cardiovasculares mayores</span></span></strong><span style="font-family: Garamond, serif;"><span style="font-size: large;">. 										<content:encoded><![CDATA[<p><span style="font-family: Garamond, serif;"><span style="font-size: large;">All hypertension is shaped upstream, within the intricate interplay of mechanisms that regulate arterial tone, sodium and water balance, and the activity of the renin–angiotensin–aldosterone system. For decades, pharmacology has acted mainly downstream, attempting to lower blood pressure once the current was already running strong. New molecules are now pointing toward a different strategy, tracing the river back toward some of its biological sources and interrupting the signal before it spreads.</span></span></p>
<p><span style="font-family: Garamond, serif;"><span style="font-size: large;">The scale of the problem is striking. <b>In 2024, an estimated 1.4 billion adults aged 30 to 79 were living with hypertension worldwide</b>, according to the World Health Organization. Yet only about 320 million had their blood pressure under control. In the United States, the most recent data indicate that roughly three out of four adults with hypertension do not have their blood pressure under control according to the 130/80 mmHg threshold.</span></span></p>
<p><span style="font-family: Garamond, serif;"><span style="font-size: large;">Within this vast population lies a particularly complex subset, resistant hypertension, which persists despite treatment with three antihypertensive agents from different classes, including a diuretic, administered at optimal or maximally tolerated doses. Some of the most compelling pharmacological innovations of recent years are being developed in this therapeutically challenging territory.</span></span></p>
<p><span style="font-family: Garamond, serif;"><span style="font-size: large;">The first pathway targets aldosterone, a hormone that promotes renal retention of sodium and water and contributes to raising blood pressure. A substantial proportion of patients with resistant hypertension have inappropriately elevated aldosterone production. Aldosterone synthase inhibitors act directly on the enzyme responsible for its synthesis rather than blocking its receptor. Their high selectivity helps preserve cortisol synthesis, overcoming one of the main obstacles that limited the development of earlier inhibitors.</span></span></p>
<p><span style="font-family: Garamond, serif;"><span style="font-size: large;"><b>Baxdrostat</b></span></span><span style="font-family: Garamond, serif;"><span style="font-size: large;"> is the first of this new generation to cross the U.S. regulatory threshold. The FDA approved it on May 15, 2026, for the treatment of hypertension in combination with other antihypertensive agents. In a phase III trial published in </span></span><em><span style="font-family: Garamond, serif;"><span style="font-size: large;">The New England Journal of Medicine</span></span></em><span style="font-family: Garamond, serif;"><span style="font-size: large;">, it produced an additional reduction in systolic blood pressure of 8.7 mmHg with 1 mg and 9.8 mmHg with 2 mg at 12 weeks, compared with placebo.</span></span></p>
<p><span style="font-family: Garamond, serif;"><span style="font-size: large;"><b>Lorundrostat</b></span></span><span style="font-family: Garamond, serif;"><span style="font-size: large;">, another selective aldosterone synthase inhibitor, is also far advanced. The FDA accepted its application for regulatory review and set December 22, 2026, as the target date for its decision. In a trial likewise published in </span></span><em><span style="font-family: Garamond, serif;"><span style="font-size: large;">The New England Journal of Medicine</span></span></em><span style="font-family: Garamond, serif;"><span style="font-size: large;">, it produced additional reductions in ambulatory systolic blood pressure of approximately 6.5 to 7.9 mmHg compared with placebo.</span></span></p>
<p><span style="font-family: Garamond, serif;"><span style="font-size: large;">The second pathway reaches an even deeper level. Instead of acting on an already formed hormone or its receptor, it intervenes in the hepatic production of angiotensinogen, the protein that initiates the renin–angiotensin cascade. <strong>Zilebesiran</strong> uses RNA interference to silence the messenger RNA encoding this protein over an extended period. It is the quiet act of pausing the command before it is ever spoken.</span></span></p>
<p><span style="font-family: Garamond, serif;"><span style="font-size: large;">Its main appeal lies in the duration of its effect. The phase III ZENITH trial administers zilebesiran as a subcutaneous injection every six months, as an add-on to standard antihypertensive therapy. The study examines <b>whether this strategy reduces the incidence of cardiovascular death, nonfatal myocardial infarction, nonfatal stroke, and heart-failure events</b>. Follow-up will continue until the number of events required to evaluate these outcomes is reached.</span></span></p>
<p><span style="font-family: Garamond, serif;"><span style="font-size: large;">The possibility of spacing administrations has clear clinical relevance. <b>Adherence is one of the major challenges of antihypertensive treatment</b>, especially when it requires daily medication for decades. Replacing hundreds of annual doses with two scheduled administrations could facilitate adherence, although its true impact on adherence and, above all, on cardiovascular events remains to be demonstrated.</span></span></p>
<p><span style="font-family: Garamond, serif;"><span style="font-size: large;">Completing this new map are <b>aprocitentan</b>, an endothelin receptor antagonist approved in the United States in 2024 for patients whose hypertension is not adequately controlled with other treatments, and investigational molecules such as <strong>ocedurenone</strong>, a nonsteroidal mineralocorticoid receptor antagonist that has shown blood-pressure reductions in patients with chronic kidney disease and uncontrolled or resistant hypertension.</span></span></p>
<p><span style="font-family: Garamond, serif;"><span style="font-size: large;">But it is essential to maintain perspective. <b>Pharmacological innovation does not displace the foundations of blood-pressure control</b>, including a healthy diet, reduced sodium intake, physical activity, maintenance of a healthy weight, and moderation of alcohol consumption. The new drugs expand the therapeutic arsenal where lifestyle measures and available treatments are not enough.</span></span></p>
<p><span style="font-family: Garamond, serif;"><span style="font-size: large;"><b>Each 5 mmHg reduction in systolic blood pressure</b> decreases the risk of major cardiovascular events by roughly 10%. A reduction of 10 mmHg therefore approaches a relative risk reduction of 20%, although the exact effect depends on the clinical context and should not be interpreted as a rigid mathematical relationship.</span></span></p>
<p><span style="font-family: Garamond, serif;"><span style="font-size: large;">Medicine is thus learning to move upstream, to search closer to the origin of the signals that raise blood pressure, and to intervene with increasing precision. The ultimate goal remains unchanged, </span></span><span style="font-family: Garamond, serif;"><span style="font-size: large;"><b>to protect</b></span></span><span style="font-family: Garamond, serif;"><span style="font-size: large;"> the heart, the brain, the kidneys, and, ultimately, the pulse that sustains us.</span></span></p>
